On the first day of each class, all students must sign the following AGREEMENT TO PARTICIPATE AND LIABILITY WAIVER in order to participate in the class. The AGREEMENT is as follows:
ALL CLASSES: I understand yoga, movement & dance, or any other physical activity involves risk and possible injury. I understand that it is my responsibility as an adult participant or parent not to participate or allow participation if there are any physical, emotional, behavioral or other problems that might compromise safe involvement. I understand that injuries can and do occur and that health insurance is a requirement. I understand that Happy Bambino, LLC does not carry medical insurance for participants and forever release the company, staff, owners, facility and equipment owners, and other related parties from the responsibility or liability for insurance deductibles, medical expenses and/or other damages incurred by my child, myself, or other family members while participating or visiting the facilities, parking area, or traveling to or at a related activity.
AUTHORIZATION OF MEDICAL CARE: In case of injury or illness during participation, I authorize and desire medical care for myself or my child at the discretion of the attending physician. I accept responsibility for all associated expenses.
PARENT RESPONSIBILITY TO SUPERVISE: When I visit Happy Bambino, LLC, or am involved in any related activity involving parental presence or participation, I understand and accept the responsibility, and any associated liability, of constantly supervising, controlling, and restricting activities as necessary to assure safety of the children I bring, and myself.
PHOTOGRAPHS AND STATEMENTS: I authorize use of my own and my child's visual image and statements in newsletters, posters, and other advertising.
VALID DATES: These agreements, waivers, and authorizations will remain valid and in force as long as and whenever my child, my self or any family member participates in any activity at or with Happy Bambino, LLC.
AGREEMENT TO PAY I understand there are no refunds, credits or guaranteed make-ups for missed classes due to personal reasons or inclement weather, and I am obligated to pay full tuition once a session begins. I accept the responsibility of paying for any damage to facility and equipment caused by myself or a family member.
FEES, DISCOUNTS AND TERMS OF PAYMENT
Terms of Payment - You must pay 100% of your balance at registration. There is an exception for childbirth classes that cost over $100 – these may be paid 50% at registration with the remainder due 30 days after classes begin. If you need more flexibility in payment, please talk with us.
Cancellations : Tuition is a session rate and non-refundable after classes begin - once the session begins, you are liable for the full session's tuition. Refunds, less a $25 administrative fee are available before the session begins in the form of store credit. Classes may be cancelled by the instructor if the minimum number of students required to register is not met. If your class is cancelled for this reason, you will receive a full refund or credit towards another class.
Transfers: Transfering your registration from a class you are currently registered for into a different session or time slot of the same class will be subject to an administrative fee equal to 10% of the class fee.
Prenatal Yoga: Students who deliver during a series or have physician orders to stop exercising may apply remaining classes to postpartum classes or receive a refund for unused classes.
INFORMATIONAL CLASSES : Instructors have agreed not to provide medical advice outside the parameters of their certifications and role in any given class. Students should seek the advice of their physician and/or other care providers for medical advice.
PRENATAL YOGA PARTICIPANTS: Your doctor, midwife, or primary care provider will need to sign an authorization for you to participate in the class. (These may be brought with you on the first day of class, or faxed directly to Happy Bambino - 608-204-6148.
